PENNSYLVANIA'S FAMILY FACILITATORS HELP INSTITUTIONALIZED CHILDREN ACCESS MEDICAID SERVICES AND OTHER RESOURCES NEEDED TO RETURN TO THEIR FAMILY HOMES
State-contracted Family Facilitators help children with complex medical needs leave and avoid institutional placements. The Facilitators coordinate with child-serving agency personnel, including the care managers at the Medicaid-funded managed care organizations, to obtain the services children need, such as in-home nurses, aides, and therapists, medical covered by the children’s Medicaid benefit.
